Gas Product Forms The following forms should be downloaded and submitted as and when necessary. New Product will require a completed Application Form and, if the Manufacturer is not the Applicant, a QA Manufacturers Declaration Form.  QA PVA Forms are required at time of product audit. The following Application Form for Gas Product is required to be completed satisfactorily before AGA can complete an assessment of an application for AGA certification.  The same Form can also be used if assessment against electrical standard/s is also required.  However, if the product requires WaterMark certification, a separate WaterMark Application Form is required (refer Water section).    QA Form - PVA (to be completed by the Certificate Holder for each audit)   QA Form - Manufacturer's Declaration (to be completed by the manufacturer if not the Certificate Holder)
